FREE FALL RISK PREVENTION PROGRAM

Falls are one of the leading causes of injury among older adults, but many falls can be prevented with the right guidance and support. We have partnered with the National Counsel on Aging to provide a FREE Fall Risk Prevention Program for patients 65 or

older to help you stay safe, confident, and independent. During a one-on-one appointment, Dr. Ashli Yoder will evaluate your personal risk factors, review your medications, discuss balance and mobility concerns, and recommend practical steps

to reduce your risk of falling. Even small changes can make a big difference in helping you remain active and enjoy the activities you love.

Thanks to available funding, qualifying participants may also receive financial assistance for safety equipment such as canes, bed supports, grab bars, and other helpful devices

designed to make everyday life safer. Whether you've experienced a fall, feel unsteady on your feet, or simply want to be proactive about your health, this program is an excellent opportunity to protect your independence.
